How do I apply for the Medicare Part D donut hole rebate?

You will receive the $250 “donut hole” rebate if you are enrolled in a Medicare prescription drug plan and have reached the coverage gap (“donut hole”) in your prescription drug benefit. You do not need to file any forms or take any action to receive the tax-free $250 check from the government.

Once you reach the “donut hole,” it may take around 45 days before you receive the rebate. If you do not receive it, and believe you are eligible for it, call 1-800-MEDICARE (1-800-633-4227).
